  • • Pharmacologic therapy (glucose lowering)

  • • Pharmacologic therapy (cardiovascular disease risk factors and renal)

  • • Use of glucose monitoring and insulin delivery devices

  • • Referral to diabetes education and medical specialists (as needed)
